Yes, you must disclose any and all known deficiencies to the buyer
on the property disclosure form. In the event you fail to tell the buyer that the roof leaks for instance, the buyer could later sue you for damages as a result of witholding material facts about the property.

Just because you disclose it, doesn't necessarily mean you will have to pay to fix it. A buyer may be willing to accept the deficiencies or may ask you to correct them, this is all negotiable between the buyer and the seller.
